Background | Procalcitonin (PCT) is a peptide precursor of the hormone calcitonin. It is produced by parafollicular cells (C cells) of the thyroid and by the neuroendocrine cells of the lung and the intestine. Under normal conditions procalcitonin is cleaved by endopeptidase to form mature hormone calcitonin. The level of procalcitonin rises during bacterial infection. It is often classed as an acute phase reactant and has become a marker of bacterial infection. Measurement of PCT can be used as a marker of severe sepsis caused by bacteria, and could help diagnose infectious complications of inflammatory bowel disease. In acute meningitis, serum PCT is useful as a biomarker for sepsis. PCT is often high in patients with chronic kidney disease and end-stage renal disease. In patients with atherosclerosis or coronary heart disease PCT is also valuable as a prognostic marker as its levels correlate with the severity of the illness. In addition, procalcitonin is reliable to monitor recurrence of medullary thyroid carcinoma. |
