Background | Growth and Differentiation Factor-15 (GDF-15), also known as macrophage inhibitory cytokine-1 (MIC-1), placental transforming growth factor-β, prostate-derived factor, nonsteroidal anti-inflammatory drug-activated gene, and placental bone morphogenetic protein, is a divergent member of the Transforming Growth Factor-β superfamily. GDF-15 is synthesized as a 40 kDa inactive precursor protein that is proteolytically cleaved to release the active C-terminal fragment, which is then secreted into the circulation as a bioactive disulfide-linked homodimer of 28 kDa. Under normal conditions, GDF-15 is expressed at high levels in the placenta with lower expression levels in a variety of tissues including the kidney, liver, lung, pancreas, and prostate. It is also expressed in the epithelium of the central nervous system. Its expression by activated macrophages is induced by inflammatory cytokines including IL-1β and TGF-β. GDF-15 has diverse biological functions with roles in inflammation, cancer, and metabolism, and it is associated with all-cause mortality and miscarriage. GDF-15 is upregulated under inflammatory conditions such as atherosclerosis or rheumatoid arthritis. Increased expression of GDF-15 is also observed in many cancers. The metabolic effects of GDF-15 are associated with the modulation of neuronal pathways important in the regulation of appetite and energy homeostasis. Increased levels of GDF-15 in the serum of individuals with advanced cancer or chronic disease are correlated with anorexia/cachexia in mice and humans. GDF-15 also exerts cardioprotective actions. In mouse models, induction of GDF-15 protects the heart from ischemia/reperfusion injury, and over-expression of GDF-15 attenuates ventricular dilation and heart failure. In humans, serum GDF-15 concentrations are associated with the risk of acute coronary syndrome as well as its prognosis. |
